Transitional
Continuum
of Care
Touchstone
Neurorecovery
Center
offers a
full
compliment
of
programs
in
home-like
settings
in order
to assist
each
individual
on their
road to
recovery.
Residents
are able
to enter
Touchstone’s
continuum
at any
point, but
most new
admissions
typically
begin
their stay
in the
Windsor or
Churchill
programs.
The
Windsor Program
utilizes
a
secured
environment
with
single-occupancy
bedrooms
to
meet
the
needs
of
residents
who
require
decreased
environmental
stimuli
or
behavior
management
as
part
of
their
treatment
focus.
The
Churchill Program
focuses
on
retraining
of
activities
of
daily
living
such
as
toileting,
bathing,
dressing,
and
grooming
while
addressing
any
residual
medical
or
neurological
concerns.
An
In-depth
Skills
Assessment
tool is
completed
routinely
on each
individual,
and as
they
progress
in their
skill
level,
they may
transition
to the
Genesis
program.
The
Genesis Program
solidifies
the
skills
already
learned
and
introduces
more
advanced
activities
of
daily
living
such
as
managing
a
home,
cooking,
and
planning,
budgeting,
and
organizing
community
outings.
When
the
individual
is ready
for the
next step,
the
Community-Based
Living
Program
awaits
just a few
miles away
from the
main
campus.
For
individuals
who can
return to
their
homes,
daytime
rehabilitation
is an
option
through
the
Touchstone
Day
program.
Day
Program
is
similar
to
the
residential
program.
It
offers
a
full
complement
of
formal
therapies
as
well
as
cognitive,
vocational,
social,
and
mood
management
activities
for
individuals
who
do
not
require
around-the-clock
care.
These
adjunct
therapies
are
included
on
each
individual’s
daily
schedule
based
on
needs
and
the
achievement
of
their
treatment
goals.
Our
Long-Term
Care
Program
Long-term
care of a
year or
more is
possible
at any of
the levels
of care,
both on
and
off-campus,
within the
continuum.
Somerset
Program
concentrates
on
maintaining
residents’
health
and
functioning
level
so
that
they
can
enjoy
the
greatest
quality
of
life
possible.
Residents
engage
in
their
daily
routines
with
as
much
independence
as
possible.
Staff
members
provide
stability
and
assistance
as
necessary
in
this
environment.
Our
Respite
Program
Some
individuals
with
neurological
impairment
can be
cared for
at home by
friends
and
family.
Being a
caregiver
can be a
challenging
and
exhausting
experience
– and it
is
important
to
periodically
make time
for a
respite to
recharge,
physically
and
emotionally.
Touchstone
offers
caregivers
the
opportunity
to bring
family
members
who meet
neurological
criteria
to
Touchstone
for as
little as
24 hours
up to 2
weeks.
While
staying at
Touchstone,
the
individual
participates
in a
variety of
activities
that suits
their
interests
and
abilities.
The staff
members at
Touchstone
welcome
both new
and
returning
respite
clients.
